An important role of support workers is to help the person decide what they need support to do, and to support these needs by following individualised plans. These plans are written in consultation with the person and their family to provide instructions to the support worker, and ensure that the support provided is responsive to the person’s needs and preferences. The emphasis of an individualised plan is placed on goals, aspirations and activities that foster the person’s participation and independence in a meaningful way.
